Payment Methods
Complete Health Wellness Group LLC (CHWG) accepts various forms of payment, including but not limited to credit and debit cards, electronic funds transfers, and other payment methods as specified on our website. Payments must be made at the time of service unless otherwise agreed upon in writing.
Service Fees, Payment Amounts, and Frequency
Fees for healthcare services will be communicated prior to the initial appointment and periodically thereafter. You acknowledge and agree that the payment amounts charged by CHWG to your Payment Method are determined by various factors, including but not limited to the fees established by your provider, the reimbursement amounts determined by your insurer, and the timing of both provider sessions and insurer payments. At the time of scheduling your appointment, CHWG will provide an estimated cost for the session based on available information; however, please note that this amount may vary due to changes in provider fees, your insurance coverage specifics, or the type of services rendered during the appointment.
Upon completion of your provider session, CHWG will initiate a charge to your Payment Method for the estimated amount. This charge may include your co-pay or an approximate fee for the service. Following your session, CHWG will submit a claim to your insurer for reimbursement, which will typically determine the final amount covered and your financial responsibility. Should there be a discrepancy between the estimated and final costs, CHWG reserves the right to adjust the amount charged to your Payment Method accordingly. The revised invoice will generally be available for your review to show any additional charges incurred after the insurer's payment determination, allowing you to remain informed of your financial obligations regarding the services received.
Insurance Responsibilities
If you are utilizing insurance, it is the member's responsibility to verify their benefits for the services provided. The information received from the health insurance company is not always accurate, and while we strive to give an estimate based on this information, discrepancies may arise. We will make every effort to resolve any issues with incorrect benefit information; however, any differences are ultimately the responsibility of the patient and/or responsible party. CHWG will assist in the submission of claims but is not responsible for determining coverage or reimbursement amounts. All copays, coinsurance, and deductibles are due at the time of service and will typically be processed on the credit card we have on file within 24 hours after the appointment.
Refund Policy
All payments received for services rendered are non-refundable, unless otherwise stipulated by applicable law. In certain circumstances, such as billing errors or overpayments, refunds may be processed at our discretion.
Outstanding Balances
Patients and/or Personal Representatives are responsible for promptly paying any outstanding balances on their accounts related to services rendered by CHWG. If an account remains unpaid for a period exceeding 30 days from the date of billing, it may be deemed delinquent. In such cases, CHWG reserves the right to initiate collection proceedings. Patients will receive written notice prior to the account being forwarded to a collection agency, providing an opportunity to settle the balance. In accordance with applicable debt collection laws, all costs incurred in the collection of unpaid balances, inclusive of attorney fees and collection agency fees, will be charged to the patient and/or responsible party. By accessing and using our services, you acknowledge and agree to these terms regarding outstanding balances.
Payment Method Updates and Available Funds
As a patient of CHWG, you are responsible for maintaining a valid and active Payment Method that is adequate to cover all charges associated with your healthcare services. It is essential that your Payment Method has sufficient funds or credit available at the time of service. You agree to promptly update your Payment Method if there are any changes, such as expiration dates or changes in your credit card information. CHWG is not liable for any fees related to insufficient funds or over-limit charges that may occur. By utilizing our services, you affirm that you understand and accept these conditions, ensuring a smooth and uninterrupted experience in receiving your healthcare services.
Changes to Payment Terms
CHWG reserves the right to modify these payment terms at any time. Any changes will be effective immediately upon posting on this Site. It is the patient's responsibility to review the payment terms periodically for any updates or changes.
